NC Pre-K Application Process
NORTH CAROLINA PRE-KINDERGARTEN APPLICATION FOR JOHNSTON COUNTY

For children who will be 4 between September 1, 2011 and August 31, 2012
The Partnership for Children is currently accepting applications for the NC Pre-K Program, a pre-kindergarten program for eligible 4-year-olds. The program is designed to better prepare children for school by using a curriculum that enhances children’s physical, social, emotional, and mental well-being.
The program is community-based, voluntary, and targets those children who are at-risk for success in school.
In order to apply for the NC Pre-K Program, the child must be 4 years old on or before August 31.
The parent/guardian must provide the following documents:

Child’s birth certificate

Child’s social security card
-
Proof of all income (current pay stubs for one month, child support, SSI, Disability, etc.) <Please see Income Guidelines>

Child’s Medicaid card (or notification of private insurance)

Child’s immunization (shot) record (Print out from doctor's office)

Child’s current 4 year old physical (the yellow physical form must be completed by the child’s doctor - MUST include vision and hearing screenings)

Proof of residence (light/water bill, lease agreement, or notarized statement from homeowner)

Three emergency contact names and phone numbers.

Verification of child’s diagnosed special needs (IEP, if applicable).
All of the documents listed above must be submitted with application.
A child WILL NOT be considered for enrollment until ALL documents are received.
NC Pre-K classrooms are located in public schools, Head Start, and in high quality child care centers.
The NC Pre-K staff are required to place children in the most appropriate site available.
NC Pre-K placements are made based on certain risk-factors, not on a first-come/first-serve basis.
Download all 3 of the following documents, gather copies of all neccessary documentation, and call for an appointment to turn in the completed application and all supporting documents. Please take the time neccessary to completey fill out your application before calling to set up an appointment. The instruction sheet will give you a complete list of everything you will need including how to set up an appointment.
